1) Approved a variation to the Homecare and Outreach –
North, Extra Care Services – Parkview Court and Grassy
Meadow Court and Specialist Support Services –
Boroughwide contract with Comfort Care Homecare
Limited to include provision of a pilot Delirium Pathway
Support Service for a five- month period from 30 January
2023 at a total cost of £146,000 (£5,840 per week); and
2) Agreed that this includes the provision to exte nd the
variation for up to one further year, delegating approval of
any extension to the Executive Director of Adult Services
and Health, in consultation with the Leader of the Council
and Cabinet Member for Health and Social Care.
Reason for decision Recommendation 1: If agreed, the recommendation will permit
a variation to the Homecare and Outreach – North, Extra Care
Services – Parkview Court and Grassy Meadow Court and
Specialist Support Services - Borough-wide contract with
Comfort Care Homecare Services that was approved by Cabinet
in March 2022. This will enable a pilot to be established that will
test a model of service provision intended to facilitate people
experiencing delirium returning to their usual place of residence
where they no longer require treatment in a hospital setting. The
pilot is one of a range of initiatives to free up hospital beds.
Concerns about managing someone with delirium can and do
lead to family carers being reluctant to accept their relative back
home, therefore resulting in longer stays in hospital than

medically necessary. The pilot is intended to address this and
will primarily comprise of a block of 196 hours of personal care
per week. The key component that makes it different from the
usual domiciliary care provision is the availability of night sitting.
Key outcome measures to determine the success of the pilot will
be:
• Reduction in the number of people being discharged to
short-term placements in care homes
• Increase in the number of people discharged to their
usual place of residence.
The pilot is fully funded by the NHS and the Council is leading
on the procurement of t he service because of its established
relationship with the proposed provider and also due to its
market management and development responsibilities under
the Care Act, 2014.
Comfort Care has been invited to deliver the pilot as it is aligned
to the ser vices that they
already provide to support hospital
discharge, namely the Bridging Care and Reablement Services.
These services are included within the scope of the contract
referred to in paragraph 1 above.
Recommendation 2: The ability to extend the d uration of the
pilot gives flexibility to allow it to continue if additional funding is
available and operational experience suggests that this option
would be beneficial to residents and Hillingdon’s health and care
system.
Procurement support the recommendation to make the variation
to the Homecare and Outreach North paper to enable the
inclusion of this provision. The variation is within the 50%
modification allowance within the regulations and satisfies the
required criteria. Given the short-term availability of the funding,
there is no other commercially or operationally viable solution
than to award to an incumbent contractor and this particular
contract represents a good alignment with existing services.
Alternative options
considered and
rejected
Recommendation 1: Officers have not considere d a
competitive tender option because of the short-term availability
of the funding, i.e., 2022/23, and the consequent short duration
of
the pilot. However, the Leader of the Council and Cabinet
Member for Health and Social Care could decide not to approve
the recommendation. This is not recommended as the funding
is only available during 2022/23 and provides an opportunity to
test a service model that has the potential to address a need that
could benefit residents and release hospital beds.
